for self-care. This includes basic tasks for day-to-day functioning,
including bathing, grooming, dressing, eating, mobility, communication
and toileting. Instrumental Activities of Daily Living, or IADLs, are
less basic but important activities like shopping, housekeeping,
personal finances, laundry and meal preparation.

How does the process work?
After speaking with you and learning about your needs, we send a
registered nurse into the home to conduct a complete assessment of the
home environment. Then, we develop a comprehensive plan of care based on
patient needs. A copy of this care plan is left in the home.
I don’t live in close proximity
to my parent. Can I still arrange care?
